i have this happen many times actually, especially with prismas where they don't put on any more layers of a color and actually start taking off color.
best thing to do is spot the issue before its too late. often you will start noticing that your pencil is no longer laying on any color BEFORE it starts flaking. In the future you will want to put down less layers to avoid this.
the way I handle it is i lay the pencil sideways and try to lay down more color, then i use a brush to mix it all together filling in the area as best I can. OR use the brush first, then try on more color/layers.
The brush kind of works like a finger when smudging. Again it depends on the area you are working with, but i will tell you i don't think you will have much hope of drastically changing how it looks per se, but just making it passable and somewhat fixed...
